A. Price Estimates

1. This price estimate is a preliminary calculation and does not constitute an official invoice or legally binding contract until a formal agreement is executed.
2. All listed prices are considered final. Negotiation of service costs is not a practice of The Glamour Team at Bella Abu Dhabi - Valiant Dubai.
3. This document is system-generated and does not require a corporate stamp for validation.
4. The provided estimate has been formulated specifically in accordance with the treatment plan deliberated during the consultation. Any modification thereto is prohibited without the express written approval of both the attending physician and the Clinic's management.
5. The final cost is subject to variation contingent upon the complexity of the case, the duration of the procedure, the quantities of materials utilized, and the location of service. Pricing is determined through an automated calculation based on these variables.
6. The price presented herein is an approximation generated by an artificial intelligence system, predicated on initial surgical requirements. The final cost may be subject to change following the finalization of all procedural elements. It is understood that treatment plans and associated costs are unique to each patient.
7. This price estimate shall remain valid for a period not to exceed thirty (30) days from the date of the client's visit.

B. Payments, Discounts, and Fees

1. Full payment for all agreed-upon services is required prior to the commencement of any treatment or service.
2. Any applied discounts are valid until the expiration of the specified offer period or for one calendar month from the date of the client's visit, whichever occurs first.
3. For all surgical procedures, payment in full is due upon the scheduling of the surgery date or no later than fifteen (15) days preceding the scheduled procedure, unless an alternative payment schedule has been explicitly stipulated in writing.
4. To secure a surgery date, a non-refundable booking fee is mandated. The fee is AED 5,000 for procedures performed within The Glamour Team at Bella Abu Dhabi - Valiant Dubai facility and AED 10,000 for procedures performed within a hospital.
5. The client shall be responsible for all applicable Value Added Taxes (VAT) and any other government-levied taxes associated with the services described herein.

C. Scope of Services for Surgical Procedures

1. An estimate for a surgical procedure may encompass the following items:
   • Anesthesia Consultation Fee
   • Operation Theatre Charges
   • Applicable Hospital Charges
   • A single (1) night of post-operative stay in the hospital, if medically required
   • A maximum of six (6) follow-up appointments, inclusive of wound care.
2. The following items are expressly excluded from the estimate, unless otherwise specified in writing:
   • Post-operative or surgical garments
   • All hematological analyses (blood tests)
   • All radiological examinations (e.g., X-rays, MRI)
   • Electrocardiogram (ECG) tests
   • Pharmaceutical prescriptions and medications
   • Polymerase Chain Reaction (PCR) or pregnancy tests
   • Post-operative lymphatic massage sessions
   • Any other items or services not explicitly listed herein.

D. Scheduling, Modifications, and Agreement

1. Acceptance of the terms herein shall be communicated via telephone to +971 50 144 3888, at which time the Estimate Number must be provided. Upon such confirmation, The Glamour Team at Bella Abu Dhabi - Valiant Dubai shall proceed with the execution of the services as stipulated.
2. Any services requested that are beyond the scope of this estimate shall incur additional charges and are contingent upon the approval of the physician and the Clinic.
3. The services shall be rendered in accordance with the schedule provided by the designated case manager or pursuant to a timeline. The Glamour Team at Bella Abu Dhabi - Valiant Dubai reserves the right to reschedule surgeries for any safety or operational reasons.
4. The selection of the hospital facility for any surgical procedure remains at the sole discretion of The Glamour Team at Bella Abu Dhabi - Valiant Dubai. The patient will be formally notified of the designated facility a minimum of five (5) days prior to the scheduled surgery date.
5. Any requested alterations to the services detailed in this estimate are not binding unless formally accepted and confirmed in writing by The Glamour Team at Bella Abu Dhabi - Valiant Dubai. Such alterations may result in additional charges, for which the client agrees to be liable upon requesting and confirming said changes.

Invoice/Bill Terms/Agreement

1. Payment Due Date: Payment is due in full upon receipt of this invoice unless otherwise stipulated in a separate written agreement. All amounts are payable upon receipt of this invoice.
2. Payment Validity: All payments made are valid for a period of three (3) months from the date of payment, after which they expire.
3. Offers & Discounts: All services booked during a promotional offer or for which a discount was applied are strictly non-refundable.
4. Late Payment: In the event of a delay in payment, a late fee of AED 200 per day will be applied for a maximum period of seven (7) days. If the invoice remains unpaid after 7 days, any scheduled treatments will be automatically canceled, and the full outstanding amount, inclusive of all accumulated fees and charges, will become immediately due and payable.
5. Payment Methods: Payments may be remitted via Bank Transfer, Credit Card (Visa, MasterCard), or Debit Card. Bank account details are available upon request.

Cancellation, Rescheduling & Refund Policy

6. Non-Cancellable Services: All services that have been booked and confirmed are considered final, fully payable, and cannot be canceled.
7. Cancellation Policy: If a service is explicitly designated as cancellable, the following notice periods are mandatory:
   • Non-Surgical Services: Cancellation must be made at least 48 hours prior to the scheduled appointment.
   • Surgical Services: Cancellation must be made a minimum of ten (10) days prior to the scheduled surgery date.
   • Failure to adhere to these timelines will result in forfeiture of any amounts paid, and the total amount will become immediately due and payable.
8. Rescheduling Fee: For surgical procedures, a fee of AED 2,500 will be applied to any request to reschedule a confirmed service.
9. Refund Policy: Refunds are subject to the following conditions:
   • A formal refund request must be submitted by filling out and signing the official clinic refund form. No refund will be processed without this document.
   • No refunds will be issued for services that have been partially executed or availed.
   • If a refund is approved, the following deductions will be applied to the refunded amount:
     • An administrative fee of 5% of the total value or AED 700, whichever is higher.
     • Bank processing fees of 5% of the total value or AED 500, whichever is higher.
     • Any other applicable booking fees or non-refundable deposits.
   • Approved refunds will be processed within 45 days from the date of approval.

Deposits

1. Service Allocation: Deposits are allocated to specific services and cannot be transferred to other services or canceled without the express approval of both the treating doctor and The Glamour Team at Bella Abu Dhabi - Valiant Dubai Management.
2. Validity: All deposits are valid for a period of three (3) months from the date of payment, after which they expire.
3. Refunds:
   • The Glamour Team at Bella Abu Dhabi - Valiant Dubai reserves the right to approve or deny refund requests at its sole discretion.
   • No refund will be issued for a deposit if any portion of the associated service has been utilized.
   • In cases where a client with an unused deposit balance wishes to obtain a refund, they may be eligible for a Gift Voucher equivalent to the remaining amount, subject to management approval.

Booking Fees, Rescheduling & Cancellations

6. Booking Fee: A booking fee of AED 200 is applied towards your service and will be deducted from the total final invoice for the consultation or treatment.
7. Rescheduling:
   • A confirmed appointment may be rescheduled a maximum of two (2) times.
   • To reschedule, a minimum notice of 48 hours prior to the scheduled appointment is required.
8. Cancellation:
   • To cancel an appointment, a minimum notice of 48 hours is required.
   • A cancellation fee of AED 100 will be applied to cover administrative and bank fees.
9. No-Show & Late Cancellation: Failure to provide 48 hours' notice for a cancellation or a no-show for a confirmed appointment will result in the forfeiture of the full amount paid (booking fee or deposit).
